The eyes are often one of the first facial features people notice, and heavy eyelids or under-eye bags can make men appear tired, stressed, or older than they feel. Male blepharoplasty, also known as men’s eyelid surgery, is a men’s procedure designed to refresh the appearance of the upper and lower eyelids while preserving masculine facial features and natural expression.
At her boutique plastic surgery practice in Westport, CT, Dr. Ellen Mahony provides personalized male blepharoplasty procedures tailored to each patient’s facial anatomy and goals. With a refined aesthetic approach and advanced surgical expertise, Dr. Mahony focuses on creating subtle, natural-looking rejuvenation that helps men look more rested, confident, and energized without appearing overdone.

What is Male Eyelid Surgery?
It’s called blepharoplasty, and it can reduce drooping eyelids, as well as puffiness or bags under the eyes. Eyelid surgery offers an ideal option for men seeking a more energized, youthful appearance without being overly noticeable.
The surgery involves removing excess fat and skin from the lids, allowing them to open again and potentially improving your line of sight. You’ll still have the same rugged good looks, just those from your younger days.

How Male Blepharoplasty is Different from Female Blepharoplasty
Dr. Mahony uses different surgical techniques for male and female eyelid surgery. This is because of the differences in both anatomy and how men and women perceive what needs to be done to refresh the eyes. For men, heavier upper eyelid skin often causes droopy eyelids that can eventually affect the field of vision.
In the upper eyelid, if ptosis (sagging) is present, Dr. Mahony will tighten the levator muscle, which has likely stretched over time. With women, the goal in the upper eyelids tends to be skin removal and fat repositioning. Women want a wider, more rounded look. Removing skin from the male eyelid can create a more feminine look, which needs to be avoided.
In the lower eyelid, if the muscle has descended, Dr. Mahony will tighten it to remove sagging. Men have thicker facial muscles, heavier eyelids, and heavier brows, and Dr. Mahony takes these factors into consideration when planning her male eyelid surgeries.
The goal is for the patient to look alert, refreshed, and younger, but the final result must look completely natural.

Many patients – men and women – are concerned about looking unnatural after plastic surgery. While my goal is always to create a natural appearance, eyelid surgery, in particular, offers an exciting boost without a glaring change. Most men report that they appear more energized or are simply told they “look great,” but friends and coworkers cannot determine what has changed. This is why I find eyelid surgery to offer great bang for your buck – you will have an improved, more youthful appearance, but people usually cannot tell that you’ve had surgery.
My goal is to make you look like a younger, more energized version of you. Eyelid surgery will not reduce your masculine appearance. In fact, it’s a great option for improving your vitality without altering the rugged good looks men want. You might be surprised to know which handsome Hollywood guys have turned back the clock without losing any of their box-office manliness.
Again, this treatment is difficult for others to pinpoint. They may verbalize that you look rested, but they won’t usually know why. Men tell me that they feel very comfortable with the results and do not get asked whether they have had plastic surgery.
Blepharoplasty is a form of eyelid surgery that consists of removing excess skin and fat, then repositioning tissues to enhance the natural contours of your eye area. If your upper eyelids hang, your line of sight can also be improved with this surgery.
If you have eyelids that droop or sag or have lower eyelid bags, you are likely to be a good candidate for blepharoplasty. Most eyelid surgery patients are in their 40s or older and have started to experience the sagging and bagginess due to the natural aging process. Before choosing eyelid surgery, you will have a one-on-one consultation with Dr. Mahony to discuss your areas of concern. She will help you decide whether eyelid surgery is the right option for you.
Most patients need to plan a week of recovery time following eyelid surgery. During this time, your incisions may be red and swollen, and you can use ice packs and eye drops to relieve discomfort. You will have a follow-up visit 3 to 4 days after surgery, at which time, your stitches will be removed. By two weeks following surgery, your bruising and swelling should have subsided completely.
As mentioned above, these are not difficult or lengthy recoveries, so you don’t need to plan for serious downtime. But you’ll still need to lay low for a few days, so make the necessary arrangements you need to at work and in other areas of your life.
You’ll need to stop taking blood thinners, aspirin, anti-inflammatory medications, and most herbal supplements for a few days before your surgery, as these can all cause excessive bleeding.
Eyelid surgery offers a solution for drooping eyelids that come with age. As your body continues to age, the natural processes will continue. You can expect to enjoy 8 to 10 years of benefits from your eyelid surgery, at which time a second surgery may be performed to restore your energized appearance.
Eyelid surgery is becoming increasingly popular among men worldwide, as they feel the need to keep up with their younger counterparts at work. Eyelid surgery does this subtly. Unless you tell others you’ve had this surgery, they’ll have no idea. They’ll just think you look somehow younger and more alert.
Eyelid surgery with Dr. Mahony only addresses the eyelids and the muscles controlling them. This surgery doesn’t affect the eyes or the patient’s vision in any way.
Dr. Mahony is a board-certified plastic surgeon and has extensive experience with eyelid surgery. Still, this is surgery, so it entails all the possible risks inherent in all surgeries: excessive bleeding, infection, reaction to anesthesia, poor incision healing, and the like.
Complications specific to eyelid surgery are:
-
Inability to fully close the eyelid
-
Eyelid infection
-
Double vision
-
Noticeable scarring
-
Malpositioned eyelids
-
Undercorrection
-
Overcorrection
-
Numbness of the eyelids
These complications are quite rare. This is a satisfying procedure for our male clients.
Recovery from this surgery is not difficult or even very painful. Dr. Mahony will prescribe prescription pain medication, but most patients don’t feel they need it, opting instead for over-the-counter options. The pain from this surgery is not acute, and most of the discomfort resolves within the first two or three days.
In the upper eyelid, Dr. Mahony makes the incision in the natural crease, effectively hiding it from view. In the lower eyelid, the incision is made along the eyelash line or on the inside of the eyelid. Both options show little if any scarring. Eyelid skin is thin, and it heals very quickly and effectively. This keeps scarring to a minimum.
