Aging, weight loss, sun exposure, and collagen depletion can gradually lead to facial hollowing, skin thinning, volume loss, and overall tissue decline. ALMI™ (Autologous Lipocyte Micronized Injection) / Microfat Transfer is an advanced biostimulation treatment that uses your body’s own living fat and biologic material to naturally restore volume, improve tissue quality, and support long-term rejuvenation.
Unlike traditional fillers that temporarily add fullness, ALMI/Microfat Transfer combines living fat cells with regenerative components to help revitalize aging tissue from within. This innovative procedure not only restores youthful contours but also supports collagen production, tissue repair, hydration, and overall skin quality using your body’s own DNA-based biologic material.
Dr. Ellen Mahony offers personalized ALMI/Microfat Transfer treatments designed to achieve natural-looking rejuvenation and long-lasting restoration.

What Is ALMI/Microfat Transfer?
ALMI/Microfat Transfer is a regenerative fat transfer procedure that uses your own adipose tissue (fat) as a natural source of volume and regenerative cells. Fat contains a rich supply of biologic and regenerative material that can help support tissue repair and rejuvenation.
During the procedure:
-
Fat is gently harvested from areas such as the abdomen or thighs
-
The fat is processed and refined into smaller particles for precise injection
-
Regenerative cellular material is separated and preserved
-
The processed fat is strategically injected into targeted areas requiring volume restoration and rejuvenation
Because the transferred fat consists of living tissue, the cells can develop a blood supply and remain in the treated area for extended periods.
How ALMI/Microfat Transfer Supports Regeneration
ALMI/Microfat Transfer goes beyond traditional fillers by combining volume restoration with regenerative support.
Potential regenerative benefits may include:
-
Improved collagen and elastin production
-
Increased skin thickness and hydration
-
Enhanced tissue quality and elasticity
-
Improved circulation and capillary growth
-
Smoother skin texture and tone
-
Long-lasting volume restoration
This treatment helps create a biologic environment that supports tissue repair and ongoing rejuvenation over time.
The Role of PRP in ALMI/Microfat Transfer
Platelet-rich plasma (PRP) is often combined with ALMI/Microfat Transfer to further support regeneration and healing. PRP contains concentrated growth factors derived from your own blood that help stimulate cellular repair and collagen production.
When combined with microfat transfer, PRP may help:
-
Enhance tissue healing
-
Improve skin quality
-
Support regenerative cell activity
-
Promote collagen production
-
Improve overall rejuvenation results
Together, fat transfer and PRP create a comprehensive regenerative treatment approach using your body’s own biologic material.

Good question! The procedures are very similar in that both use your body’s own cells to stimulate deep tissue regeneration and the important biologic elements that “reverse” aging, such as collagen, hyaluronic acid, and capillaries. ALMI is different in that we include your own adipose fat cells to create fullness in areas where many patients want it, most often to plump sunken, drawn cheeks and veiny aging hands. The regenerative cells from ALMI actually replace damaged tissue and help rebuild it. This is more of a foundational process that creates new cellular material. In comparison, PRP stimulates repair processes that reverse aging. When used together, these synergistic procedures are extremely powerful in their impact on aging.
While I can’t speak for any other doctors, in my practice, we choose to offer a variety of solutions we consider effective and safe – whether they are biologic or manufactured – because each patient has unique concerns, budget and needs. In addition, some doctors may not be trained in ALMI so they aren’t able to recommend it in their practice. I took intensive courses in both the ALMI and PRP procedures to stay on the cutting edge of what I believe is an amazing area of DNA-based rejuvenation.
The Microfat transfer procedure typically takes 1 to 2 hours once you arrive for your appointment. This entails getting you checked in and settled in your private treatment room. A local anesthetic may be administered to the area from which we will harvest fat cells as well as the area into which those cells will be transferred. It may take a short while for the local anesthetic to reach full effect. The fat transfer process then involves three phases; the phase in which we harvest fat cells, the phase in which we process harvested cells and enhance them with the biological matrix, and the phase in which the solution is introduced into the area of concern.
Fat transfer procedures are an alternative to dermal fillers, a common wrinkle-reducing procedure. To date, no singular cosmetic technique can address all of the signs of aging. The ALMI-Microfat transfer can reinstate more youthful contouring in various areas of the face but you may still wish to address other age-related concerns. This can be done as needed with various resurfacing treatments, a thread lift, or even a face or neck lift. Dr. Mahony meets one-on-one with each patient to fully ascertain their needs and expectations. Treatment plans are then customized accordingly.
The fat transfer technique is intended to restore volume to the face or hands for years. While aging is not a process we can stop, it is possible to achieve lasting results from a single ALMI microfat transfer, followed by good skincare and sun-protective habits.
After the ALMI Microfat transfer procedure, patients may experience some degree of redness, swelling, and bruising. These natural side effects usually last only two or three days. They can be managed with ice or cool compresses, applied for approximately 20 minutes at a time a few times a day. Patients may also take arnica or apply an arnica gel to areas of tenderness or swelling.
The ALMI Microfat transfer procedure may require a short period of downtime for recovery. Mild bruising, swelling, and tenderness may occur in both the harvest area and the recipient area of the face or body. Post-treatment side effects may be mild enough for some patients to resume normal light activities within a day or two. Conversely, side effects may fade gradually over 2 to 3 weeks. Even in such instances, patients may feel comfortable enough to return to work and other daily activities within a few days.
